With the score tied and the bases loaded, Phillies reliever Orion Kerkering fielded a soft comebacker from Andy Pages, then hesitated. Instead of the sure out at first, Kerkering threw home — and air-mailed catcher J.T. Realmuto. The error allowed Hyeseong Kim to score the winning run, giving Los Angeles a 3–1 series win and ending Philadelphia’s season in shocking fashion.

According to MLB.com, the Phillies’ loss marked the first postseason series in MLB history to end on a walk-off error. The mistake capped a frustrating offensive series for Philadelphia, which mustered only five total runs over four games.
For the Dodgers, it’s their fourth NLCS appearance in six years. For the Phillies, it’s another October heartbreak — and a meltdown that will linger all winter.
