An old concern has suddenly returned for Shohei Ohtani, and it is one that the Los Angeles Dodgers can hardly afford to ignore.

Ohtani was in the middle of another standout performance Thursday against the Pittsburgh Pirates when the situation took an unexpected turn. The Dodgers star exited the game during the seventh inning at PNC Park, with Santiago Espinal entering as a pinch hitter in his place. Up to that point, Ohtani had been dominant at the plate, finishing his day 2-for-2 with a home run and two walks.

What initially appeared to be a routine substitution quickly became more significant when the Dodgers announced that Ohtani had been removed because of inflammation in his left knee.

A Familiar Medical Concern

That update immediately raised concerns because of Ohtani’s long-documented history with the joint. Since childhood, he has dealt with a bipartite patella in his left knee, a congenital condition in which the kneecap remains divided into two separate pieces rather than fusing into a single bone.

While many athletes never experience major issues with the condition, Ohtani has had periodic flare-ups throughout his baseball career. The most notable episode came in 2019 during his time with the Los Angeles Angels. That knee problem ultimately required surgery and ended his season prematurely.

Because of that history, any mention of inflammation involving the same knee is likely to attract immediate attention from both the Dodgers and baseball fans around the league.

A Strong Dodgers Season Interrupted

The timing also adds another layer of concern. Ohtani has been remarkably durable this season despite carrying one of the most demanding workloads in professional sports. He has appeared in 63 of the Dodgers’ first 69 games as a hitter while also working his way back into a full role as a starting pitcher.

Balancing those responsibilities is challenging under ideal circumstances, and even a minor physical setback can become more complicated when a player is contributing on both sides of the game. Ohtani has consistently been one of the club’s most productive players, making any absence especially significant as the season progresses.

Dodgers Await Further Answers

The Dodgers have already been cautious with Ohtani throughout the year, carefully managing his workload as he continues his return to pitching. That approach reflects how valuable he is to the organization and how important his long-term health remains to their championship aspirations.

For now, the club has not indicated the severity of the issue, leaving plenty of uncertainty surrounding what comes next. Inflammation does not automatically signal a major injury, but given Ohtani’s medical history, it is understandable why the announcement generated concern.

The Dodgers will likely be hoping this proves to be a short-term flare-up rather than the beginning of a larger problem. Still, whenever Ohtani’s left knee becomes part of the conversation, memories of past setbacks are never far behind. With one of baseball’s most unique talents at the center of the story, every update will be watched closely in the days ahead.
